Dyslexia Support Surrey may collect and process the following information:

• child or young person’s name, date of birth, age and school year
• parent/carer names and contact details
• school details and relevant school contact information
• educational history and learning concerns
• information from parent/carer questionnaires
• information from school, where consent has been given
• previous reports, assessments or professional information shared by parents/carers
• teaching notes, assessment notes and observations
• assessment results and report information
• information relating to strengths, needs, support and progress
• payment and invoicing information
• relevant safeguarding or wellbeing information, where necessary

Some of this information may include sensitive information about a child’s learning, development, health or wellbeing.

Why is infomration collected?

Information is collected and used to:

• respond to enquiries
• arrange specialist teaching or diagnostic assessment
• understand the learner’s strengths, needs and learning profile
• plan and provide appropriate specialist teaching
• carry out diagnostic dyslexia assessments
• write reports and recommendations
• liaise with schools or other professionals, where consent has been given
• manage bookings, invoices and payments
• meet professional, legal, safeguarding and insurance responsibilities

Lawful basis for using information

Dyslexia Support Surrey uses personal information because it is necessary to provide specialist teaching and assessment services, manage bookings and communicate with families.

Where sensitive information is processed, this is done only where necessary for educational, professional, safeguarding or legal purposes, and with appropriate care.

Consent will be requested where information needs to be shared with schools or other professionals, except where there is a safeguarding concern or legal requirement to share information.

Who this infomration may be shared with?

Information will only be shared where necessary and appropriate.

This may include:

• parents/carers
• the learner, where appropriate
• schools or SENCos, where consent has been given
• other professionals, where consent has been given
• safeguarding services, where there is a safeguarding concern
• professional bodies, insurers or legal advisers, if required

Reports or personal information will not be shared with schools or other professionals without parent/carer consent, unless there is a safeguarding concern or legal obligation.

How information is stored?

Personal information is stored securely using password-protected electronic systems and/or secure paper records.

Paper records are kept securely and are not accessible to unauthorised people.

Reports and documents are shared securely wherever possible.

How long information is kept for

Information is kept only for as long as it is needed for professional, legal, safeguarding, insurance or business purposes.

Social media and examples of work

Anonymised examples of teaching resources or activities may occasionally be shared for professional purposes. No identifying information will be shared without separate parent/carer consent.

Children’s faces, full names, school names or identifying personal information will not be shared as part of standard practice.

Your rights

You have the right to:

• ask what personal information is held about you or your child
• ask for incorrect information to be corrected
• ask for information to be deleted, where appropriate
• ask for information to be restricted or object to how it is used, where applicable
• withdraw consent where consent has been used as the basis for sharing information
• complain if you are unhappy with how your information is handled

Some information may need to be retained for legal, safeguarding, professional or insurance reasons.
